Why can't I create new signed certificates on a Smoothwall CF2008?

I am trying to create new signed certificates on a Corporate Firewall 2008 Smoothwall machine and I am getting an error that says

Error - Unable to create certificate, possibly due to a duplicate Subject.

I think this is a flavor of OpenSwan. I can find nothing with this error text on Smoothwall's support site.

I do have shell access if you want to suggest any command line work.

firewall certificate smoothwall create openswan
